Passport Information

ALL PERSONS traveling between the United States and any other country, are required to present a passport or other valid travel document to enter or re-enter the United States.

All Passport Applicants MUST appear in Person
(including minors under age 16)

Applications for passports may be obtained at the Kitsap County Clerk's Office or you may download and fill them out from: - look for "Forms" at the right of the page.

All applications MUST be filled out with black ink.

To obtain a passport you must have the following items:

  • Proof of Citizenship: certified copy of Birth Certificate, previous passport, or Certificate of Naturalization
  • 1 official passport photos, 2 X 2 inches (Now available in the Clerk's Office for $10)
  • Valid photo identification
  • Applicant's social security number

Special requirements for Children under age 16
Both parents or child's legal guardian must:

  • Present evidence of child's U.S. citizenship and
  • Present evidence that they are the parents or guardian and
  • Show valid personal identification and
  • Sign and take oath before an authorized passport acceptance agent
  • If either parent is unable to appear, the absent parent must provide a notarized statement of consent, which includes the child's name and date of birth, authorizing passport issuance and a photo copy of the front and back of their picture ID.

Passport Renewal:

IF you have an existing passport or passport card that was ISSUED LESS THAN 15 YEARS AGO and AFTER you turned 16 and certain other conditions apply, you can renew by mail using Form DS-82 and avoid the $25 processing fee.  These forms CANNOT be processed by the Clerk's Office, but must be mailed directly to the address on the form.  If you do prefer to have the Clerk's Office process your renewal it will be necessary for you to fill out form DS-11 and pay the $25 processing fee.

Renewals for Children cannot be done by mail.  The requirements for renewal are the same as those listed above under "Special Requirements for Children . . .", except that only a copy of the citizenship evidence (birth or naturalization certificate, for example) is needed.

The Passport Card:

Customers only now have the option to apply for a Passport Card which can be used for land and sea travel between the Unites States and Canada, Mexico, Bermuda and the Caribbean.  For more information visit:
